Making a referral to YASP
YASP has an open referral system for all its services. Anyone can make a referral. Please download our YASP Referral Pack for details of our services. This includes details of the services we provide, who we can work with and how to make a referral.
Please download and print out the YASP Referral Form. This can be posted or faxed. The Word version of the YASP Referral Form can be completed electronically and e-mailed.

Self-referral
You can make a referral for yourself. You can come into our Café and talk to someone. The YASP Café is at 832 Stockport Road, Levenshulme, Manchester M19 3AW. We are open Monday to Friday from 10am-4pm.
You can phone and discuss our services with someone at YASP. You will be asked for your details when you phone and we will complete a form with you if you decide you would like to be referred.

Making a referral to the Making Tracks Project
Specialist GPs are able to make a referral to the Making Tracks Project. This is a different way to access YASP services.
